Customized JobCare Applications:
FireMed will provide a special membership packet just for your employees. We will include a custom application form that includes your employer information, and discount coupons for the special rate you offer to your employees. It will also contain information about the FireMed program and answers to most of the questions they will have. All other questions can be directed to the helpful FireMed staff.
Payroll Deduction Authorization Form:
This form is simply to allow each employee to authorize the company to deduct the employee's portion of the FireMed fee from payroll and submit their payment to FireMed.
First Aid Booklet:

This small instructional booklet is available to all members so that they will understand their role in assisting any medical emergency. For on-the-job emergencies, it is usually a fellow employee who has the opportunity to respond and assist the victim.

This Emergency Medical Plan Poster educates all employees in correct protocol in a medical emergency. It also reinforces their benefits and privileges as FireMed members. We intent the poster to be a functional document which includes company-specific information relative to emergencies that occur on site.
